Quick Answer

Which FDACS forms include a notarial section?

Send the exact unsigned FDACS form you are using, including the form number and revision date. We identify the notarial section on that page. Two currently accessible revisions of FDACS-16023 do different things. Detailed form guides are linked below this umbrella page.

Why the revision matters

Check the form number, the revision date, the instructions, the signing language, and the visible notarial section. Do not infer a notarization requirement from the title or the form number alone.

Two currently accessible copies of FDACS-16023, Affidavit of Experience, do different things. Confirm which version the receiving agency requires. Full analysis: FDACS-16023 Affidavit of Experience.

How to identify the form number and revision

FDACS-16018, as downloaded on 2026-10-05, carries both Rev. 03/26 and Rev. 11/20 stamps. Send the exact unsigned form before scheduling.

  • Read the footer form number.
  • Find Rev. or a month and year near the form number.
  • Count the pages.
  • Notice whether the page sits inside a larger application packet.

Standalone 16023 and the copy inside the Class C packet

Do not write that FDACS Form 16023 always requires a notary. The revision on the page you will sign controls the session. Deep dive: FDACS-16023 Affidavit of Experience.

FDACS-16023 Rev. 10/16

The standalone PDF prints a notary instruction and a jurat. The applicant is the signer.

FDACS-16023 Rev. 11/2020 inside the Class C packet

The copy embedded in the current Class C application packet uses a perjury declaration. The signature block reviewed on 2026-10-05 does not print a notary certificate.

That affidavit is not the personal-inquiry waiver

Standalone FDACS-16023 is the Affidavit of Experience. It is not the personal-inquiry waiver. The license application has its own execution section, and that waiver can use a different method from the experience affidavit.

FDACS-16055 partner, officer, or LLC-member change

FDACS-16055, Application for Change in Agency Partner, Corporate Officer, or LLC Member, revision 05/18, tells the new partner, officer, or member not to sign until in front of a notary, and it prints that notarial section. One optional attachment is a notarized resignation letter. Dollar amounts printed on the FDACS instructions, including $50 and $42 figures, are FDACS fees. They are not iRemoteNotary session fees. Deep dive: FDACS-16055 agency partner, officer, or LLC member change.

FDACS-16016 private-investigator intern completion

FDACS-16016, Termination/Completion of Sponsorship for Private Investigator Intern, Rev. 02/08, is the sponsor's sworn affidavit and this revision prints a notary jurat. The instructions say the sponsor completes it within 15 days of termination or completion. The sponsor is the signer. iRemoteNotary does not sponsor the intern. Paired guide: FDACS-16016 and 16017 intern termination or completion.

FDACS-16017 recovery-intern completion

FDACS-16017, Termination/Completion of Sponsorship for Recovery Agent Intern, Rev. 02/08, is the Class EE sponsor's sworn affidavit. This revision prints a notarial section. The sponsor is the signer. Send that revision if it is the form you are using. Paired guide: FDACS-16016 and 16017.

FDACS-16013 Temporary Class G

FDACS-16013, Temporary Class G License Agency Character Certification, Rev. 03/08, is signed by the licensed agency owner or manager. This revision prints a notary jurat. The notary does not perform the evaluation and does not decide Class G eligibility. Deep dive: FDACS-16013 Temporary Class G character certification.

FDACS-16026 sponsor-form discrepancy

Rev. 01/14 on the standalone URL

The standalone PDF prints two jurats.

Rev. 04/2020 inside the Class C and Class CC packets

That embedded version shows signatures and a section 837.06 warning. It does not print those jurats.

The Class C instructions still say notarized

FDACS controls the filing

The Division decides completeness and licensure. iRemoteNotary does not verify employment, sponsor an intern, approve a Class G finding, or decide licensing eligibility.

Investigation agencies and security or recovery agencies can use the same request path. See investigator and sponsor sessions and security and recovery agency forms.

What to have ready

Have the unsigned PDF, the form number, the revision, the signer's legal name, and a government ID. A signer can be outside the United States when the commissioning state's rules allow the appearance. That appearance rule is on signer appearing from outside the United States.

Frequently asked questions

What is the difference between the two FDACS-16023 revisions?

Which personal-inquiry pages include a notarial section?

What if the form and the instructions disagree?

Use the page the person will sign. If that page has no notarial section, do not schedule a notarization of a certificate that is not there.

What should I send before scheduling?

The unsigned form, the form number, the revision date, and the name of the person who will sign.

Does iRemoteNotary approve the FDACS application?

No. The Division decides completeness and licensure. The notary completes only the notarial section printed on the page.
