Is my Personal Inquiry Waiver the same as FDACS-16009?
Not always. Many packets use a Personal Inquiry Waiver and Verification section that is not the standalone 16009 PDF. Check the form number and revision.
Identify whether you hold standalone FDACS-16009, an application PIW section, or FDACS-16055 Section IX before you schedule a notary session.

It depends which instrument you are holding. Standalone FDACS-16009, Personal Inquiry Waiver Authority for Release of Information (Rev. 02/08), prints a notarial section on the reviewed revision. Many current license application packets use a Personal Inquiry Waiver and Verification section executed under penalties of perjury without that traditional jurat. FDACS-16055 Section IX still uses a Notarization Statement and jurat. Do not generalize the older standalone notarial block to every current package.
| Item | Detail |
|---|---|
| Standalone form | FDACS-16009 Rev. 02/08 |
| Common packet title | Personal Inquiry Waiver and Verification |
| 16055 title | Personal Inquiry Waiver and Notarization Statement |
| Risk | Same everyday name, different execution methods |
Send the exact unsigned page—including form number and revision—before scheduling. FDACS materials for this workflow may differ by form revision or application package. Confirm the exact form number, revision date and current filing instructions before signing. Eligibility for remote notarization and recipient acceptance are separate questions. The Division of Licensing decides completeness and licensure.
Chapter 493 applications require a personal inquiry waiver so the Division can investigate background and related records. Florida Statutes § 493.6105 lists a personal inquiry waiver among application contents. The everyday title “Personal Inquiry Waiver” appears on more than one instrument.
Applicants, new officers/members, and coordinators commonly complete whichever version their packet or standalone form provides.
| Where it appears | Execution on reviewed materials | Notary certificate |

| FDACS-16009 Rev. 02/08 standalone | Notarial section on reviewed revision | Yes |
| FDACS-16018 / 16038 application sections | Personal Inquiry Waiver and Verification — penalties of perjury | No on those application sections |
| FDACS-16055 Section IX | Personal Inquiry Waiver and Notarization Statement | Yes |

Experience affidavits are a different form: FDACS-16023. Do not confuse the experience affidavit with any personal-inquiry page.
When your exact page prints a traditional notarial act, bring government ID that satisfies the online notary’s commissioning-state rules. When the page is only a perjury declaration, do not schedule a notarization of a missing certificate.
