Quick Answer

What is a Minor Group Travel Consent Form?

A Minor Group Travel Consent Form documents permission for a child to travel with a school, sports team, church, camp, tour, or other organized group. It includes the group leader, organization, event, destination, dates, signatures, contact information.

School permission is not parental border consent, not an airline or cruise form, and not a government process. Last verified 2026-09-14.

Check for an official form first

Check for an official form first. Some airlines, cruise lines, schools, and government offices require their own document.

Use this sample only when no official form applies. It is not an airline form, a cruise form, or a government form.

If the child is sailing without a parent or legal guardian, existing cruise-line non-parent forms still control. If a destination government template or exit process applies, use that process. See the group travel guide.

What is a Minor Group Travel Consent Form?

A Minor Group Travel Consent Form documents permission for a child to travel with a school, sports team, church, camp, tour, or other organized group.

School permission is not parental border consent. It is not an airline or cruise form. It is not a government process.

Read the group travel scenario guide first.

When to use this form

This sample may help for a school, sports team, church or religious group, music group, youth organization, camp, or organized tour when a generic parental travel letter is accepted and no official form controls.

Who should sign the form?

The parent or legal guardian with authority signs. The group leader does not become a legal guardian by leading the trip.

What information should be included?

Identify the child, parent or legal guardian, organization or group, group leader, leader contact, destination, travel dates, transportation information, lodging or contact where useful, parent emergency contact, and travel permission. Use the optional checkbox if a separate medical authorization is attached.

Does this form need to be notarized?

Not always. Whether notarization is required depends on the carrier, destination, government authority, court order, or organization receiving the document.

If the applicable carrier, destination, court order, government authority or recipient requires notarization, leave the signature unsigned until the notarial act can be completed correctly. There is no universal school-trip notarization form.

Remote online notarization caution

Notarization requirement is not remote online notarization acceptance. A school, coach, or border officer may still want paper originals.

Original or electronic documents

Some recipients still want paper originals. Confirm whether an electronic notarization, a printed copy, or an original paper document is required before relying on a remote session or a printout.

If someone asks for notarization

Requirements vary by carrier, destination, and receiving authority. An official form or different wording may be required.

The receiving party may ask for an acknowledgment, a jurat, witnesses, or no notarization at all. If notarization is required, do not sign the letter until the notarization.

A completed notarization is not permission to board, to enter a country, or to skip an official government form.

When this sample should not be used

Do not treat this as the school's own permission slip, an airline Unaccompanied Minor form, a cruise-line official form, or a government exit permit.

Do not automatically bundle full medical authority. Optionally check that a separate medical authorization is attached and use the medical consent for travel sample.

School permission is a different document

School permission is not parental border consent. It is not an airline or cruise form. It is not a government process.

If the child is sailing without a parent or legal guardian, existing cruise-line non-parent forms still control. If a destination government template or exit process applies, use that process.

Official forms and government requirements

If an official carrier form, government template, or government process applies, use that instead of this sample. iRemoteNotary does not rehost official PDFs or process portals.

Read the group travel scenario guide first. Use the Family Travel Center and Child Travel Requirement Checker before relying on a generic letter.

For a non-group letter, see the general minor travel consent sample. If medical authority is requested, use the medical consent for travel sample.

Common mistakes

  • Treating school permission as international border consent.
  • Ignoring carrier non-parent rules on a group cruise or flight.
  • Bundling unlimited medical authority into the group letter.
  • Assuming there is one universal school-trip notarization form.

Frequently asked questions

Is a school permission slip the same as this letter?

No. School or activity permission, international border consent, carrier requirements, and government exit permission are different documents.

Should medical permission be in the group letter?

Not automatically. Use the optional attached-medical checkbox and the separate medical consent sample when that authority is actually requested.

Does every youth trip need notarization?

No. There is no universal school-trip notarization form. Notarize only if the applicable recipient or rule requires it.

Do I sign the form before the notary?

If a jurat is required, do not sign the document in advance. The signer must sign or affirm the statement as required during the notarization.

Sources and Official References

The following primary sources support the legal and procedural information in this guide. Requirements can change, so confirm current rules with the receiving organization before relying on a notarized document.

Keep group, carrier, and border documents separate

School or activity permission is not international border consent, not a carrier form, and not a government exit process. Do not automatically bundle full medical authority into a group travel letter.
