Florida law requires a certified-process-server applicant to obtain and file a certificate of good conduct. Some circuits publish a notarized certificate. Use the form from the circuit that will receive your application, and send that unsigned page for review.
Section 48.29 requires the certificate of good conduct to be filed with the application. It states the criminal-history content of that certificate. It does not print a notary certificate. iRemoteNotary does not decide those facts and does not issue the certificate.
These are the circuits whose official materials were reviewed. They are examples, not a statewide form.
The Fifteenth Judicial Circuit publishes a Certificate of Good Conduct with a before-me jurat, plus a separate penalty-of-perjury application oath in the same packet. The certificate filename date is 08-2026.
The Third Judicial Circuit application includes an application jurat and a separate certificate jurat. The filename date is 7-8-2024.
The Eleventh Judicial Circuit new-applicant steps, updated 2026-06-12, describe identity, the fee receipt, the FAPPS course, and a background check. They do not reproduce the notarized certificate. Do not generalize that description to circuits that were not reviewed. Follow the circuit or sheriff program that will receive your application.
