Quick Answer

Which Florida Retirement System forms require notarization?

FR-11 directs the member to sign in the presence of a notary and includes a separate employer-approval step. SA-1 uses notarized member execution and, when the member is married and selects Pension Plan Option 1 or 2, a notarized spouse acknowledgment.

FR-13 includes a notarized disability-retirement application signature. SA-2 addresses the Investment Plan disability spousal-acknowledgment process. FRS-11o is the option-selection form; do not assume FRS-11o itself requires notarization merely because its packet directs a member to complete SA-1.

FRS form numbers identify different jobs. Member execution, spouse acknowledgment, employer approval, disability documentation, and retirement-option selection should not be collapsed into one signature rule.

This guide explains form mechanics, not which benefit option to choose. For other retirement systems and document types, use the Retirement, 401(k), TSP & Pension Notary Document Center.

FRS Forms at a Glance

Use the current official PDF and its instructions. The summary below does not replace the form packet.

FRS formPurpose covered hereNotary / approval pointImportant limit
FR-11Pension Plan application for service retirementMember signs in the presence of a notary; employer approval is a separate part of the processNotarization does not replace employer certification or FRS review
SA-1Pension Plan spousal acknowledgment connected to an Option 1 or 2 selectionMember signature is notarized; spouse acknowledgment applies if the member is married and selects Option 1 or 2Do not require a spouse signature solely because the member is married without checking the selected option and current form
FR-13Application for disability retirementMember signs the application in the presence of a notaryMedical, employment, and eligibility materials remain separate requirements
SA-2Investment Plan disability retirement spousal acknowledgmentFollow the current SA-2 member and spouse acknowledgment blocks and their notarial instructionsDo not substitute Pension Plan SA-1 for the Investment Plan disability form
FRS-11oRetirement option selectionTreat it as the option-selection form; the packet directs completion of SA-1 when applicableDo not claim FRS-11o itself requires notarization unless the current form says so

A notary should identify the certificate and signer on the presented form, not infer a requirement from another form in the packet.

FR-11: Member Notarization and Employer Approval

FR-11 is the FRS Pension Plan Application for Service Retirement. Its member certification directs the applicant to sign in the presence of a notary.

Employer approval is a separate step. A completed notarial certificate does not certify the member's final employment information, replace the employer's approval, or approve the retirement application.

Before signing, confirm that you have the current FR-11 and that all blanks intended to be completed before notarization are complete.

SA-1: When the Member and Spouse Sign

SA-1 is tied to the Pension Plan benefit-option process. The member's execution is notarized.

The spouse acknowledgment applies when the member is married and selects Option 1 or Option 2. In that situation, follow the current SA-1 instructions for the spouse's notarized signature. Do not turn this into a rule that every married FRS member's spouse signs every retirement form.

The spouse acknowledgment records receipt of the information stated on the form. A notary does not explain the financial effect of the selected option or advise the member or spouse whether to sign.

FRS Disability Retirement Notary: FR-13 and SA-2

FR-13 is the member's disability-retirement application. The official form includes a member signature in the presence of a notary. Notarization is only one part of a disability packet; medical reports, employer information, deadlines, and program eligibility remain separate.

SA-2 is the Investment Plan disability retirement spousal-acknowledgment form. Use SA-2—not Pension Plan SA-1—for the Investment Plan disability acknowledgment identified by the packet. Follow the current form's member signature and spouse acknowledgment instructions, including the notarial blocks that apply.

Do not assume every disability-related attachment needs notarization. Check each current form by number and edition.

FRS-11o Selects an Option; SA-1 Handles the Acknowledgment

FRS-11o records the member's Pension Plan option selection. The packet may direct the member to complete and return SA-1 when Option 1 or 2 and marital status make that acknowledgment applicable.

That packet relationship does not prove that FRS-11o itself requires notarization. Look for a notarial certificate and signing instruction on the current FRS-11o before claiming that it must be notarized.

This Guide Does Not Recommend Options 1–4

Options 1–4 can affect payment duration, survivor benefits, and monthly benefit amounts. This page does not recommend an option, compare financial outcomes, or tell a spouse whether to acknowledge an election.

Direct benefit-election questions to official FRS counseling resources or an appropriately qualified financial or legal professional. The notary's role is limited to the authorized notarial act.

Confirm FRS Acceptance Before Using Remote Online Notarization

Recipient confirmation is required. The FRS PDFs reviewed establish form-specific notarization instructions, but they do not establish a blanket FRS policy accepting remote online notarization for every form, signature, electronic original, or submission channel.

Remote online notarization is performed under the law of the online notary's commissioning state. Before booking, ask FRS whether it will accept RON for the exact form number and edition you are submitting.

  • Confirm the exact form number and current edition
  • Ask whether FRS accepts remote online notarization for that form
  • Confirm whether an electronic original PDF is accepted
  • Ask whether the member and spouse may use separate sessions if both signatures are required
  • Confirm where employer approval belongs in the FR-11 process
  • Verify the submission method and deadline

FRS confirmed remote acceptance?

After FRS confirms that it accepts remote online notarization for your exact form, signer configuration, and submission method, you may start a secure session with a commissioned U.S. online notary.

Frequently asked questions

Does FR-11 require notarization?

The official FR-11 directs the member to sign in the presence of a notary. Employer approval is a separate requirement and is not replaced by notarization.

When does a spouse sign SA-1?

Follow the current SA-1 when the member is married and selects Pension Plan Option 1 or Option 2. Do not assume every married member needs a spouse signature for every option or form.

Does FR-13 require a notary?

The official FR-13 disability-retirement application includes a member signature in the presence of a notary. Other disability packet requirements remain separate.

Is SA-2 the same as SA-1?

No. SA-1 addresses the Pension Plan option acknowledgment described on that form. SA-2 addresses the Investment Plan disability retirement spousal-acknowledgment process.

Does FRS-11o itself require notarization?

Do not assume that it does. FRS-11o is the option-selection form, while its packet may direct the member to complete SA-1. Check the current FRS-11o for its own signing instructions.

Does FRS accept remote online notarization?

This guide does not claim blanket FRS acceptance. Ask FRS about the exact form, edition, signer configuration, electronic original, and submission method before booking.

Can a notary advise me which FRS option to select?

No. A notary cannot recommend Options 1–4 or provide financial advice. Use official FRS counseling or an appropriately qualified adviser for benefit-election questions.
